Dr. Melissa Alonzo, PT, DPT
Doctor of Physical Therapy
Melissa Alonzo earned a Doctorate in Physical Therapy from East Tennessee State University in 2017. She worked at sports medicine departments in hospitals in Steamboat Springs and Mammoth Lakes before opening her private practice in 2021. She is an active volunteer for US Ski & Snowboarding, having worked with Olympic Athletes in Snowboard Cross and Alpine Ski Racing. Melissa is the Strength Coach and Physio for Mammoth Track Club, the local professional Track Team.

Mammoth Physio is an out-of-network provider with all commercial insurances, except for Medicare. Depending on your insurance benefits, you may qualify for reimbursement. Mammoth Physio can issue a “superbill”, which is used to submit a claim with your insurance company. Payment is due at the time of service.

When speaking with your insurance provider, it's important to ask the following questions:
Does my plan provide out-of-network benefits?
What percentage of the cost will be covered?
Is there a deductible or coinsurance for out-of-network providers?

No. The California Direct Access Law for Physical Therapy enables physical therapists to evaluate, diagnose, and treat patients without first obtaining a referral from another healthcare provider. This law reduces costs and redundancy, and time to see a provider by removing the requirement to see a physician first. You can also see a physical therapist for wellness services, meaning you don’t need to be actively injured to benefit from the services provided by PT’s.

Wellness services include: Manual Therapy & Neuromuscular Re-education for maintenance of chronic or acute musculoskeletal dysfunctions. Strength Testing with Force Plate Technology Strength Training Programming: In Person and Virtual Home Exercise Programming.

At Mammoth Physio, an Initial Evaluation service is required to establish care at our facility. A physical therapist will conduct a thorough evaluation and develop a personalized plan of care, which may include Manual Therapy, Therapeutic Exercise, and Neuromuscular Re-education. We use a battery of tests and measures in order to create a snapshot of your overall physical health, including force plate testing, manual muscle testing, range of motion measurements and orthopedic special tests. You can expect to be issued a Home Exercise Program at this visit in order for you to start your management of your issue immediately.

It is best to wear comfortable clothing that allows you to move freely. Avoid wearing restrictive clothing such as jeans or tight-fitting clothing. It is also important to wear shoes that provide good support and cushioning.
